To prevent the spread of COVID-19 and to reduce the potential risk of exposure to our players, coaches, and families we are conducting a simple screening. Your participation is important to help us take precautionary measures to protect you and everyone in the Club.

We request you complete this screening EVERY time your player comes to practice. It should be completed BEFORE arriving for practice at the Soccer Plex. A SEPARATE form is required for each player.

Based on your response, you will be informed if you can participate in practice.
